To me - the so-called Tomahawk Steak is just a Rib Eye with the bone.
Can anyone give some idea of when this Tomahawk term started?

I would say that the 'meat people' benefit significantly from this term. I did some analysis of a few 'Tomahawk steaks' and found that the bone was about 25% of the gross weight - and yet the price was about the same as a Rib Eye WITHOUT the bone. Nice price for soup bones!
