To me - the so-called Tomahawk Steak is just a Rib Eye with the bone.

Can anyone give some idea of when this Tomahawk term started?

I would say that the 'meat people' benefit significantly from this term. I did some analysis of a few 'Tomahawk steaks' and found that the bone was about 25% of the gross weight - and yet the price was about the same as a Rib Eye WITHOUT the bone. Nice price for soup bones! :)
 
A ribeye with the bone in is known as a Cowboy steak around here. A tomahawk is an extra long bone and can be seen on a ribeye or a veal chop even. YMMV.

But when did the term 'Tomahawk' begin?

And wonderig - has anyone noticed a significant difference in price (lower) when buying a Tomahawk cf. usual Rib Eye?
